Here are a few details we would like you to know about the process.
Patient Forms
Our goal is to streamline your first visit in every way possible, so we are pleased to announce that we are a paper-free office. Instead of downloading forms, printing them and filling them out at home, you can fill them out online here at your convenience before your visit. Or, you can plan to arrive about 15 minutes before your appointment and use our iPad kiosk to complete them.
What to Bring with You
In addition to the forms you’ll find online, we will need you to bring your photo ID and any insurance cards that you will use. When you call to arrange your first visit, our team will talk to you about transferring x-rays from your previous dentist.
Meeting the Team
When you arrive, you will be greeted warmly and checked in by Leurica at our front desk. During your brief wait, we invite you to relax in our comfortable reception area and help yourself to bottled water or a cup of coffee.
At your appointed time, we will show you to your comfortable treatment room where you will meet one of our hygienists, Betsy, Kelly, or Tammy. They will talk to you about any concerns you have and then perform a thorough dental cleaning. If we find signs of problems developing, we will work together to develop a plan for treatment going forward.
Before you leave our office, we will discuss insurance and financing options and arrange future appointments as needed.
